3 out of 5 — An established approach, with room to improve consistency or effectiveness.
5 out of 5 — Every part of the statement is consistently met, with evidence of sustained effectiveness.
A policy or isolated example is not enough for 5 out of 5. Select a lower score where practice is new, inconsistent or dependent on one person.
